Health and Medical Sciences

Congratulations on receiving an offer for a place in a UWA Health and Medical Sciences course.

There are important compulsory health and legal screening requirements that you must now commence.

Deadlines and how to submit your documents
Medical screening must commence as soon as you accept your offer. Your submission of the Infection Control Form (signed off by your doctor) is required and must be received prior to the deadline specified on the Document Pack.

Document submission is via Sonia Online, which enables you to upload and review all pre-enrolment requirements.  Later in your studies you will also use Sonia Online to view and manage your placement details.

1: Infection Control & MRSA Declaration

It is of the utmost importance that you meet the compulsory infection control requirements. Book a doctor's appointment immediately upon accepting your offer because clearance can take time. For instance, if you have not been previously vaccinated for Hepatitis B, it can take at least 8 months to complete immunisations and confirm immunity.

Details of the requirements, along with the Infection Control Form and other documents you need are provided in the Document Pack.

The UWA Medical Centre on Crawley Campus is recommended. The Centre GPs understand the infection control requirements for students and provide assistance should you need to speak with an immunisation specialist. Make an appointment online or call +61 8 6488 2118.

If you attend another GP, you must submit all of your test results and vaccination records, in addition to your Infection Control Form and MRSA form.

2: National Police Certificate (NPC)

All students will need to obtain a clear National Police Certificate. Applications can be made online or in person at participating Australia Post Outlets.

Certificates not issued by WA Police are accepted provided it has been issued by an organisation accredited by the Australian Criminal Intelligence Commission.

3: WA Department of Health Criminal Record Screening (CRS) Clearance Card

As part of your course, you will undertake placements within WA Health sites and are required (under the Criminal Record Screening Guidelines) to consent to the screening of any criminal records. The application form is included in the Document Pack.

If you have received your National Police Certificate (issued less than 12 months ago), attach a certified copy to your CRS application, and you will not be required to pay the fee.

4: First-Aid (Provide First Aid) Certificate (not applicable to Pharmacy or Social Work students)

You will be required to provide proof of successfully completing an Australian First Aid Course (accreditation number HLTAID003 or HLTAID011).

If you cannot complete the course by the deadline, submit evidence that you have booked and paid for the course. Following completion of your course, submit a copy of the Statement of Attainment as soon as it is available.

5: Inherent Requirements Acknowledgement (Master of Pharmacy and Doctor of Optometry students only)

Inherent requirements are abilities that students must possess to successfully complete the Master of Pharmacy course. These abilities are reflected in the achievement of the course learning outcomes and are necessary to fulfil the role of a pharmacy student and to prepare for pharmacy practice.

It is essential that you read and understanding the requirements, and ensure that you possess the abilities required to successfully complete the Master of Pharmacy course.
